Parent Information from Nurse  Office hours, permissions, immunization requirements, etc.
Emergency Medical Authorization  Required to authorize treatment for students while at school
General Medication Form  Required for students who require Inhalers or Epipens at school

Registration

 
Registration Form   Required to enroll new student for the 2017-2018 school year
Re-Registration 2017-2018  Required to re-enroll students for the 2016-2017 school year
Health History  Required for registration
Legal Custody  Required for registration - update when any changes
Public School District  Required annually for registration - update when any changes
2017-2018 Supply List  To be reviewed by students and parents before school begins

Internet Policy  All parents and students must read annually
Internet Access Agreement  All parents and students must sign annually (after reading policy)

Communications Permissions  Parents sign to authorize school to contact you by email, text message and phot permissions
